Full-time position (40 hours per week). Works Monday through Friday 8:00 am to 5:00 pm. Provides quality administrative and clerical services for program staff and assists Senior Life Solutions patients with care needs. Will contribute to a positive team dynamic and excellent customer service. Ensures patient care needs are met while at the program. Takes patient vital signs as directed. Assists patients in performing the sit to stand test as directed. Participates in treatment team meetings and coordination of care. Responsible for contacting and obtaining authorization from each insurance company to ensure Senior Life Solutions services will be covered. Assess patient problems on the phone, triages calls, discusses with medical provider and returns calls. Provides transportation to patients in a provided vehicle, as applicable. Experience in care to older adults in hospital or nursing facility environment preferred. High school diploma or GED required. CNA, CMA, or LPN certification required. Valid driver’s license required.
