On-call position. Works as needed Monday through Friday 8:00 am to 4:30 pm. Utilizes the nursing process to provide and direct quality patient care. Provides leadership when working with team members in maintaining standards of professional nursing practice. Participates with the interdisciplinary team to develop the plan of care and supports Home Health and Hospice philosophies in caring for clients. Promotes health and wellness through education, clinical practice, and home care for the ill and disabled. Completes assessments/reassessments, pertinent documentation, plan of care, and evaluations on time. Makes visits according to orders. Communicates with practitioner/charge nurse to keep them informed of changes in patient condition. Hospice experience and some lead nursing or supervisory experience preferred. Must have compassion, patience, and empathy for the patients and families being served. Takes responsibility for knowledge of death and dying process, grief, bereavement, and nursing management of a hospice/home health patient. Must be a graduate from an approved Professional Nursing program. Certification in Basic Life Support is required.
